Did WOJ establish that the pub opened in 1994? I didn't think that it was every mentioned in the books. All I can remember Harry saying is that the pub is a fixture in the magic community. The t-shirt says Official Neutral Territory under the Unseelie Accords since 1994. To me that means 1994 is when he gained Neutral status not when he opened shop in Chicago. I think it is likely that the Unseelie Incursion combined with things heating up in Chicago prompted Mac to get the pub declared neutral.

So in order to repay a favor you have to act in accordance with the debt-holder's wishes. If the debt-holder dies and the debt is not able to be passed along his/her bloodlines it might be possible for Mab to clear her debt by acting in accordance with the debt-holder's perceived wishes (governed by the same internal rules that apply to all Fae). For example, maybe Mab could pay a wergild to a charity supported by the debt-holder.

A bigger worldview would be nice, but I get the feeling that Harry will still be localized to the Chicago area for the main stories. Maybe Mab can cook something up that will allow him to travel by air.

Idiocy knows no borders and will be found wherever people congregate. If magic was real (as in the Dresdenverse) there would be severe repercussions because magic has consequences. Imagine all the idiots who try to summon something or want to jump ahead to the "cool stuff" like making fireballs without putting in the discipline and training first. As much as I would love to be a wizard, I do not want our society to discover magic (in the Dresdenverse form).

That is a great summary of why the Dresden series is awesome Kindler. I agree with all of it. I love the fact that two different wizards could both use a fire spell and get very different results due to the way they tap into their power and form the spell. I love that this series has crossed over into so many different genres (horror, political, detective, heist, etc) while still remaining true to the characters and the overall themes of the series.

I don't have the answer (this would make a great convention question for Jim), but I think there has to be some mechanism of clearing the debt. Obligation is a part of the Fae's nature. I'm sure a way has been worked out to cover this potential scenario.

She had become something else entirely, something very, very bad.

So six views:
1. Drug Addict Molly
2. Mother Molly with children
3. Warden Molly
4. Normal Molly (possibly apprenticed to Harry)
5. Non-supernatural normal Molly
6. Warlock Molly

I am going with whatever causes Harry the most pain. To me that is either Warden Molly who is firmly set against Harry (actively hates maybe) or Warlock Molly who is allied with Mirror Harry, but is evil and twisted.

To add to your statement about Harry being poor, if I may. Staying underfunded is, also, a form of behavior control. A way to deny himself some luxuries and comforts.

Actually, this one is interesting. Molly getting in trouble in Proven Guilty is an indirect result of Harry picking up Lasciel's Coin. Harry had consciously avoided Michael ever since, to the point where he hadn't seen him in a couple of years (especially odd because they're pretty much best bros). Because Harry wasn't around, he didn't pick up any of the signs from Molly, and she didn't have the opportunity to casually talk to him about it. Mab even lampshades it in Cold Days (page 480 on Nook):
Harry's absence, especially after his frequent presence and mysterious example, is one of the things that engineered Molly's circumstances. So, in Mirror, Mirror, if he didn't pick up the Coin, maybe Molly never went warlock in the first place. I'd actually like that, because the Mirrorverse might be a bit boring if everything is objectively worse in every regard, you know?

I see either two things happening. Either Harry is more involved in Molly's training from an earlier point and thus prevents her from going warlock due to not picking up the coin. On the other hand, Harry could withdraw due to Susans (supposed death/turning) and once again misses the signs of Molly's training leading to a similar outcome as the prime reality or a worse outcome in which she is either dead or joined with supernatural bad guys in her quest for power.

If I remember correctly, Morgan initially thinks that the skinwalker started tracking him when he went through Arizona or Nevada during his flight from the Wardens. He seems to think that the skinwalker is chasing him because he is wounded thus easy prey and to add a wizards power to his own.

Harry on the other hand leans towards the Shaggy is working for someone answer. He mentions the Shaggy was a diversion for Madeline to escape theory to someone (I think). Lara or Murphy maybe? I'm going to have to do a reread at some point soon.
