Cautionary tales - anyone broken their game yet?

One of the best features of FATE 3.0 based games is the open-source, hack-friendly nature.  Of course, that aspect can be compelled as well as invoked. ;) I'd love to hear some horror stories or cautionary tales of great ideas that turned out badly.  Before I let my group get all hack happy with the system, I'd like to know what NOT to do, or at least what to be cautious of.

The system is hack-friendly, but it also has a number of not immediately obvious interrelationships. While I don't have a horror story to offer here per se, I would encourage folks to try running it "vanilla" before adding tweaks of their own. See how what's there works first.  Don't assume it's all obvious without playing.
